Spring 2008 and Fall 2008 Season Forms

Please download and review the following documents before agreeing to the waivers during registration.  You do NOT need to print them out for submission, but please read them carefully before agreeing to them on-line.

• Athletic Code of Conduct
• Attendance Policy

The following Forms MUST be printed out and brought to the Parent Meeting or first day of practice.   Athletes may be asked to sit out from practice until all forms and registration are in.

• 2008 US Rowing Waiver  - must be submitted only once per calendar year.
• 2008 Sport Candidate and Permission Form-  must be submitted EACH season
• Pittsford Crew MD Form -  For the Spring 2008 Season, we must have an MD Form with a physical dated AFTER 8/1/06.  For the  Fall 2008 Season, the physcial must be dated after 8/1/07.
